Birth & Death Certificate department is responsible for issuance of Death Certificate to the citizens of the Bidhannagar Municipal Corporation (BMC) area upon application for cases of death within the jurisdiction of Bidhannagar Municipal Corporation.

All the applications for Death or Change of Name in the Death Certificate are collected at Borough Offices.

Applicant needs to submit an application through the specific application form to apply for Death Certificate at the concerned Borough Office or at counter of the BMC head office counter.

For change of name in the Birth Certificate there is no application form. An applicant needs to submit an application along with the required supporting documents.

Preliminary check of the documents and the application is done at the time of application submission.

In case of unavailability of all the required documents the application is not received.

On successful preliminary check of all the documents a challan is issued to the applicant for deposit of fees.

The applicant needs to deposit Rs.100/- for one copy of the Death and additional Rs.50/- for an additional copy of the Death Certificate as per his/her requirement.

An applicant can apply for duplicate copy of the Death Certificate by paying Rs. 160/- (Rs. 100/- for certificate +  Rs. 60/- as searching fees)

The application is sent to the Death certificate department from counter for verification.

In case of any discrepancy the application is rejected and the reason is mentioned in the application and signed by the department staff and returned to the respective Borough or BMC head office counter.

Respective Borough or BMC head office counter communicates the reason for rejection to the applicant and asks to comply for application processing.

The hospitals send the Death Report on daily basis to department. The details are entered to the software and a registration number is generated by the software. This data is used upon receipt of application for Death Certificate.

The application is verified against with the hospital data and other submitted documents.

The details of the application is entered to the Death  certificate software and the Death  Certificate is printed, signed by the department staff and placed before the Joint Municipal Commissioner (JMC) for final signoff.

On sign off by the JMC the certificate is sent to the concerned Borough or Counter.

Separate registers are maintained for Death Certificate at the concerned Borough and BMC head office counter. The registration number and details of the certificate is recorded in the concerned register and the certificate is handed over to the respective applicant.

  4. In case of no legal heir, First Class Magistrate affidavit copy on Stamp paper of Rs. 10 (original)

After one month but before one year of death at Home

  1. Doctor Report (photo copy)
  2. Cremation Certificate original and photo copy
  3. ID proof of the applicant/legal heir (self-attested)
  4. Notarized copy for delay registration of death of Rs.10 (original)
  5. In case of no legal heir, First Class Magistrate affidavit copy on Stamp paper of Rs. 10 (original)

After one year of Death at Home

  1. SDO (at DM Office Barasat birth & Death Department) permission for Rajarhat area
  2. SDO permission (at DJ-4, Sector-2, Salt Lake, Kolkata-91)

Within one Month of Death at Hospital

  1. Doctor Certificate (photo copy)
  2. Cremation Certificate original and photo copy
  3. ID proof of legal haire (self-attested)
  4. In case of no legal heir, First Class Magistrate affidavit copy on Stamp paper of Rs. 10 (original)

After one month but before one year of death at Hospital

  1. Doctor Report (photo copy)
  2. Cremation Certificate original and photo copy
  3. ID proof of the applicant/legal heir (self-attested)
  4. In case of no legal heir, First Class Magistrate affidavit copy on Stamp paper of Rs. 10 (original)

After one year of Death at Hospital

  1. Doctor Report (photo copy)
  2. Cremation Certificate original and photo copy

Extra copy of Death Certificate if stolen or lost

  1. Forwarding Letter to Registrar birth & death department, BMC, Kolkata-106
  2. Photo copy of death certificate if any
  3. General Diary copy from PS
  4. ID Proof of either parent in case of death ID proof of legal haire

Note: Death certificate will be prepared as per the Cremation Certificate.

Fees for Death Certificate

  1. Within One year 1st Copy Rs.100 and additional Copy Rs.50 (maximum 5 copies)
  2. After One year Searching fee Rs.60 and Late fees of Rs.2/year along with Rs.100 for 1st Copy of death certificate and Rs.50 additional Copy (maximum 5 copies).
